Welcome to the Canes! Please fill out the form below in its entirety to officially register for the Spring 2023 Baseball Season. Note that it is REQUIRED for ALL players to fill out this form (including current or previous players). The Directors of the UA Canes Southwest and the above signed Player and Parents/Guardians hereby enter into the following agreement:

Select Baseball Team Fees and Structure

The purpose of the Canes program is to provide youth the privilege and opportunity to participate in a high quality baseball program exhibiting the finest of ability, sportsmanship, integrity, patriotism and citizenship. Our program will prepare your athlete to play baseball at a higher level through innovative training and productive game days.

As part of joining the Canes Southwest, I agree to abide by the following:

  1. The Fee structure of the program for the Spring 2023 Season is listed below. Payment plan available please read below. This does not include uniforms for new players (we will have a Wilson fitting day)

  2. Season will go from Feb - May 30th weekend, with a make up in June. Total Cost for the spring season is $1395

  3. 12U - 14U Players A deposit/check of $280 is due upon execution of this agreement. The remaining Balance will be paid in 5 equal payments on the 1st of each month ($280).  All payment plans must be secured via credit card 1st of each month(Jan-June)

  4. Alternative payment plans can be arranged.

  5. There are no refunds once committing to the program. By signing above you are committing to a full season of baseball with Canes SW LLC and all fees associated with the program as stated above.

  6. Spring season will have 8, and an option for the end of season trip.

Medical /Liability Release Form

I/We the parents of the above named candidate for a position on an UA Canes Southwest Select Baseball Team hereby give our approval to participate in any and all Canes activities, training, and practice sessions.

I/We know the participation in baseball may result in serious injury and protective equipment does not prevent injuries to players, and do hereby waive, release, absolve indemnify and agree to hold harmless Canes Southwest LLC, Its Directors, owners, landlords, agents organizers, sponsors, instructors, supervisors or affiliates as well as any participants from any claims arising out of injury to my/our child whether the result of negligence or for any other cause, except to the extent and in the amount covered by accident or liability insurance. I understand that baseball is a dangerous sport and assume all risks associated with practicing at the EvoShield Canes Southwest program. I affirm that my child is in good physical condition and covered by health accident insurance independently.

As a parent and or guardian I hereby give my permission for the participant to be transported for emergency medical care. I also hereby consent for emergency medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. This care may be given under whatever conditions necessary to preserve life, limb or well being of my dependent.
